You will receive an immediate personalized report that outlines your health risks and offers suggestions to help you do something about them. Your Personal Health Record Your Personal Health Record (PHR) lets you track your entire health story in one place – where you’ve been, where you are and, just as important, where you’re heading.§ Information in your record comes from you and from your health plan carriers. Information from Your Health Plan Carriers Your health plan carriers will add any information that comes from your medical claims. That means you’ll always have up-to-date information about any care you receive under your health plan. When you take the Health Assessment, information from the Assessment will also be added to your PHR, making your PHR even more comprehensive and useful. Information from You Adding information yourself to your Personal Health Record is completely voluntary. You can record information that only you know – for example, your allergies and any past illnesses.
