Program InformationUPS Healthy Connections - Informed Choices The health and well-being of UPS people is a priority at UPS. UPS Healthy Connections Informed Choices is offered through your UPS health plan at no cost to you. The program works on different levels to give you the information and support you need to get the most value from your health care benefits, become a more informed health care consumer, and improve your overall health and wellness. Who Can Participate?
Program Components
Private and Confidential The UPS Healthy Connection Informed Choices program abides by all health plan privacy policies and federal and state laws. Under no circumstances will your protected health information be used in a way not otherwise explained in the UPS Notice of Health Plans Privacy Practice. This information will be used only as necessary to administer the Plan and this Program. UPS Healthy Connections Informed Choices includes condition management (also commonly referred to as disease management) programs. If you or your covered family member has one of the following conditions, you may qualify for our Condition Management Program:
A health coach will work one-to-one with you to answer questions about your condition and work toward achieving personal health goals. The Condition Management program also enables you to ask for a health coach to contact you by telephone if you have been diagnosed with any of the four chronic conditions (asthma, CAD, diabetes or CHF). Making the request to be contacted is your first step to better control of your condition and toward better overall health. To learn more about this opportunity, click here. Once you enter the program, you will partner with the same health coach throughout your program participation. Hospital Admissions and Discharge Support An upcoming hospital stay for a treatment procedure or medical test can be a source of stress. Good preparation and understanding of what is going to happen can lighten the load. That’s why we will have an enhanced case manager call you before and after your hospital stay. Or, if you have questions about your diagnosis, hospitalization or recovery, you can initiate the call by using the toll-free number on the back of your health plan ID card. Before your hospital stay, this registered nurse will reach out to you to answer any questions you may have or help you think of what to ask your doctor, and can even explain what will happen during the procedure and your entire hospitalization. Learn how to plan for your stay and how to reduce risks and improve your chances for better outcomes. Even if you were hospitalized for an emergency, any time you or a covered family member is released from the hospital, an enhanced case manager will reach out to you to make sure you understand your follow-up instructions, coordinate any home care that you may need, and to see how your recovery is going. Any time you have questions; you can contact your health plan and ask to speak with your nurse case manager.
