Personal Health Record

Get ready with your Personal Health Record
Organize, understand and use your health information, and keep track of all the details about your health.

 

Your Personal Health Record (PHR) can help you:

  • Keep all your health information in one place – Your processed medical and prescription claims information and the results from your completed Health Assessment are automatically loaded into your PHR. You can add more information that only you know, such as:
    • your family history
    • your allergies
    • past illnesses
    • non-prescription (over-the-counter) vitamins and supplements you take
    • any low-cost prescription medications you have chosen to pay for outside your current health plan
    • notes to remind yourself of the reasons for a visit, your doctor’s directions and other important information you want to remember
  • Improve the quality of care you receive – Keeping you and your doctors up to date on your medical history, condition, prescriptions and allergies can help you get the best care possible during routine visits or an emergency.
    • Consider printing a copy for your doctor.
    • Keep a copy in a safe place at home and carry one with you (in your wallet or purse) in case you need emergency care.

When a medical issue or an opportunity to improve your care arises, the computerized system will automatically send a secure alert or a reminder to you or your doctor to help you avoid potentially serious health problems and take better care of yourself.

  • Reduce or eliminate wasted time and money – Documenting tests, screenings, procedures and immunizations you’ve received can help protect against duplication, unnecessary cost and potentially serious complications.

  • Track your progress and success – By using the special Health Tracker features of your PHR, you can chart where you stand in certain critical health areas. Right now, Health Trackers are available for:
    • blood pressure (also called hypertension)
    • blood glucose (blood sugar levels, which can detect diabetes or prediabetes)
    • hemoglobin A1C (an important blood level for people with diabetes)
    • LDL cholesterol (a type of blood cholesterol that is linked to heart problems)
    • weight (excess weight can directly impact your health in many ways)
    • asthma
    • physical exercise
    • total cholesterol
    • creatinine
    • HDL
    • triglycerides

To access the Personal Health Record, visit Aetna’s secure Web site.
UPS has chosen Aetna to provide the secure online Personal Health Record to all UPSers and their eligible family members who are enrolled in UPS-administered health care plans. This includes BlueCross BlueShield, UnitedHealthcare, CIGNA, Humana and Aetna members.

Free - provided to you and your eligible dependents as part of the UPS health benefits program
Fast and easy - takes just minutes to set up, and is easy to access and update
Convenient - online and available to you 24/7
Responsive - If any aspect of your health needs attention, the PHR will automatically issue a secure alert to you and your doctor to help avoid any potentially serious health problems
Secure - The data in your Personal Health Record is stored on Aetna’s secure Web site and kept strictly confidential in accordance with health plan privacy policies and federal and state laws

 

 

The Personal Health Record is available to UPS employees and under-65 retirees, their spouses and covered dependents age 18 or older who are enrolled in a UPS-administered health care plan. A Personal Health Record for dependents under the age of 18 can be accessed through the UPS employee’s health management account. “Spouse” includes a traditional spouse, same-sex domestic partner, or civil union partner.

If Kaiser is your current medical plan administrator, you are also eligible for the Personal Health Record and the Simple Steps to a Healthier Life Health Assessment provided through Aetna at www.aetna.com.
