UPS Healthy Connections - Informed Choices

The health and well-being of UPS people is a priority at UPS. UPS Healthy Connections – Informed Choices is offered through your UPS health plan at no cost to you. The program works on different levels to give you the information and support you need to get the most value from your health care benefits, become a more informed health care consumer, and improve your overall health and wellness.

Who Can Participate?

  • UPSers and their dependents enrolled in the Flexible Benefits Plan.
  • Pre-age-65 retirees and their dependents enrolled in the UPS Retired Employees' Health Care Plan (REHCP).

Program Components

  • This Web site – Get professional information on healthy living, links to reliable resources, and valuable health reference materials. UPS employees also have access to this site through www.UPSers.com under the My Life and Career tab. Add these sites to your "favorites" folder for quick reference.
  • A health coach – Get help managing your chronic condition from a health coach, who can provide extra direction and support.
  • Enhanced case manager – When an enhanced case manager is notified by you or your health carrier that you have a scheduled hospital admission, we will contact you to help you prepare for your procedure, answer any questions and ensure a smooth recovery.
  • Health care quality initiatives

Private and Confidential

The UPS Healthy Connections – Informed Choices program abides by all health plan privacy policies and federal and state laws. Under no circumstances will your protected health information be used in a way not otherwise explained in the UPS Notice of Health Plans Privacy Practice. This information will be used only as necessary to administer the Plan and this Program.

Certain UPS Corporate Health Care function employees are responsible for providing certain necessary Plan administration services. These employees will have access to your protected health information (PHI), as needed, in order to provide these services. Only those employees who are responsible for health plan administration are able to access your PHI and only as necessary to perform payment functions and/or health care operations of the plan. Their access, at the individual plan participant level, is rarely needed. And, for many participants, it is never exercised. These employees may access your information in order to review an appeal you have filed with the Plan, to audit the proper handling of your claims by a carrier or vendor according to Plan design and payment criteria, as authorized by you, or to monitor a carrier's administration of a wellness program, including but not limited to those provided under the UPS Healthy Connections-Informed Choices program related to health coaching, case management or the incentive program.

Condition Management

UPS Healthy Connections – Informed Choices includes condition management (also commonly referred to as disease management) programs. If you or your covered family member has one of the following conditions, you may qualify for our Condition Management Program:

  • Asthma
  • Diabetes
  • Heart-related conditions: coronary artery disease or congestive heart failure
  • High blood pressure / Hypertension
  • High cholesterol
  • Weight management / Obesity
  • Chronic pain: arthritis, back pain, neck pain

    You may also reach out to a health coach if you failed to meet any of the CDL or DOT exam requirements and would like to improve your health and reinstate your commercial motor vehicle driving privileges.

A health coach will work one-to-one with you to answer questions about your condition and work toward achieving personal health goals. The Condition Management program also enables you to ask for a health coach to contact you by telephone if you have been diagnosed with any of the four chronic conditions (asthma, CAD, diabetes or CHF). Making the request to be contacted is your first step to better control of your condition and toward better overall health. To learn more about this opportunity, click here.

Hospital Admissions and Discharge Support

An upcoming hospital stay for a treatment procedure or medical test can be a source of stress. Good preparation and understanding of what is going to happen can lighten the load. That’s why we will have an enhanced case manager call you before and after your hospital stay. Or, if you have questions about your diagnosis, hospitalization or recovery, you can initiate the call by using the toll-free number on the back of your health plan ID card.  

Before your hospital stay, this registered nurse will reach out to you to answer any questions you may have or help you think of what to ask your doctor, and can even explain what will happen during the procedure and your entire hospitalization. Learn how to plan for your stay and how to reduce risks and improve your chances for better outcomes. 

Even if you were hospitalized for an emergency, any time you or a covered family member is released from the hospital, an enhanced case manager will reach out to you to make sure you understand your follow-up instructions, coordinate any home care that you may need, and to see how your recovery is going.
