The Health Coach

If you have a health risk or chronic or complex medical condition, you may be contacted by a health coach RN and offered special support and services as a part of this program.




Your health coach can:

  • Talk to you about your overall health and tell you about programs and resources that can most benefit you or a family member.
  • Provide information and resources that will help you get the most from any program(s) you decide to enroll in.
  • Help you follow your doctor's treatment plan.
  • Offer you a personalized condition management program if you have asthma, diabetes, coronary artery disease or congestive heart failure.
  • Provide referrals to dieticians, counselors and other health professionals, who can help you quit smoking, manage stress or lose weight.
  • Connect you to benefit resources such as behavioral health services and Solutions, a program that includes the Employee Assistance Program (EAP) and Work Life Services.

Not all eligible plan participants will be contacted in this portion of the program. Only a person whose records show that they may benefit from these services will be contacted. If you receive a call from a health coach, you have been identified as someone who could best benefit from a health coach resource. If you feel you need medical attention, please contact your physician or go to a treatment facility, such as a hospital.

The Condition Management program also offers you the opportunity to ask for a health coach to contact you directly if you’ve been diagnosed with:

  • Asthma
  • Diabetes
  • A heart-related condition such as coronary artery disease or congestive heart failure

A health coach can also help you if you suffer from:

  • High blood pressure
  • High cholesterol
  • Obesity
  • Chronic pain such as arthritis, back or neck pain

A Healthy Relationship

Your relationship with your doctor is important. The UPS Healthy Connections – Informed Choices program is designed to support that relationship – not replace it. A health coach can give you tips on how to work with your doctor more effectively, however only your doctor can diagnose or treat a health condition.

A Program for Everyone

The UPS Healthy Connections – Informed Choices web site has something for everyone. Use this web site to find health care information, resources and tools that interest you. Even if you're otherwise healthy, you may have a family history of disease, or have a family member who might benefit from being better informed on health issues. And remember, if you're contacted by a health coach, this means you've been identified as someone who could benefit from personal support and program services.
