An upcoming hospital stay for a treatment procedure or medical test can be a source of stress. Good preparation and understanding of what is going to happen can lighten the load. That’s why we will have an Enhanced Case Manager call you before and after your hospital stay. Or, if you have questions about your diagnosis, hospitalization or recovery, you can initiate the call by using the toll-free number on the back of your health plan ID card.  

Before your hospital stay, this registered nurse will reach out to you to answer any questions you may have or help you think of what to ask your doctor, and can even explain what will happen during the procedure and your entire hospitalization. Learn how to plan for your stay and how to reduce risks and improve your chances for better outcomes. 

Even if you were hospitalized for an emergency, any time you or a covered family member is released from the hospital, an Enhanced Case Manager will reach out to you to make sure you understand your follow-up instructions, coordinate any home care that you may need, and to see how your recovery is going. 

Any time you have questions; you can contact your health plan and ask to speak with your nurse case manager.
