2012 UPS Wellness Incentives
Earn cash for improving your health

When you’re motivated to take better care of yourself, you can enjoy a better quality of life and improved productivity. UPS offers wellness incentives to reward you for doing all you can to help lower health care costs.

Eligibility to earn rewards
Employees and/or eligible spouses must be enrolled in medical coverage under The Flexible Benefits Plan at the time the requirements for each health reward are completed. No other dependents are eligible for health rewards.

Health rewards
You and your spouse may receive up to two health rewards each, in the amount of $100 per reward. Rewards are paid in the form of cash cards, to be used for any purpose you choose. The first $100 health reward is for completing the Health Assessment. Then, you and your spouse can each earn another $100 health reward for completing a Weight Watchers® program or a defined exercise program. That means you and your spouse can earn a total of $400 in health rewards in 2012.

To receive 2012 health rewards, you must complete each incentive’s requirements and provide confirmation of completion (as indicated below) no later than December 31, 2012.

1st reward: complete your Health Assessment
You must complete the Health Assessment in 2012 before you can earn the second 2012 health reward. To access the Health Assessment online, visit your medical carrier’s Web site at either www.Aetna.com or www.MyUHC.com and follow the instructions for completing your Health Assessment. If you live in California or Hawaii and are covered by the Kaiser HMO, you must register at the Aetna Web site to access your Health Assessment and Personal Health Record (PHR).

If you completed a Health Assessment in 2011, you are required only to update the information in your Health Assessment; it is not necessary to complete a new Health Assessment. Follow the instructions on your carrier’s site.

Obtaining your 1st health reward after completing the Health Assessment
After you have taken the Health Assessment, your first health reward will be processed as follows, depending on your medical carrier. If your coverage is administered by: 

  • Aetna or Kaiser, your first health reward will be processed automatically by the Wellness Program Administrator. No further confirmation of completion is needed for your first health reward.  
  • UnitedHealthcare, you will be notified by mail or email that you have earned the first health reward. You must provide confirmation of completion by responding to the notice as soon as possible, but no later than December 31, 2012. Then, your health reward will be processed by the Wellness Program Administrator.

2nd reward: complete a Weight Watchers or defined exercise program
As with the first 2012 health reward, you must complete the requirements for the second health reward and provide confirmation of completion (as described below) by December 31, 2012. You are welcome to complete both a Weight Watchers and a defined exercise program, but you will earn a second health reward only for the one you complete first, based on the date the Wellness Program Administrator receives confirmation of completion.

Requirements for completing a Weight Watchers service 
For purposes of earning the second 2012 health reward, you must have completed attendance and/or participation in at least 10 sessions or weeks of one of the following:  

  • Weight Watchers at Work meetings
  • Local Weight Watchers Online
  • Weight Watchers Online
  • Weight Watchers At Home Kit

Obtaining your 2nd health reward after completing a Weight Watchers service
Begin by downloading the appropriate UPS Wellness Program–Weight Watchers Completion form based on your medical carrier:

Then, follow the steps below, depending on the Weight Watchers service you’re using:

  • If you have completed at least 10 Weight Watchers at Work Meetings or Local Meetings, present the completed form to your local Weight Watchers service provider for certification of attendance and his or her signature. Then mail the form to Weight Watchers at the address indicated on the form.  
  • If you have completed at least 10 weeks in the Weight Watchers Online service, print the Account Status page located within “My Profile” and your Weight Tracker Summary. Mail all three items, plus a copy of your purchase receipt, to Weight Watchers at the address indicated on the form.
  • If you have completed at least 10 weeks using the Weight Watchers At Home Kit, print and complete the form, send a copy of your purchase receipt plus a copy of your daily diary to the address indicated on the form.  

Please be sure to mail your completed form in a timely fashion for processing your 2012 health reward. Note: Any forms received after December 31, 2012, will not be counted toward the requirements for a second 2012 health reward.

Weight Watchers will notify the Wellness Program Administrator monthly of the names of UPS employees and spouses who have completed a Weight Watchers service. Only simple data, including the service completion date, will be transmitted. No other personal information, such as your weight or the amount of weight lost, will be provided. Your second health reward will be processed by the Administrator as long as you have completed the Health Assessment. Requirements for completing a defined exercise program
To earn the second health reward, another choice is to complete 1,200 minutes of any number of approved physical activities. These include (but aren’t limited to) running, walking, playing tennis, and cleaning the house. By getting up and moving, you’ll improve your mind and body through cardiovascular activities, strength training, and/or toning. Up to 30 minutes of approved exercise per day will count toward your goal of 1,200 minutes. So, for example, if you walk 30 minutes a day for 4 days each week, you can earn your second health reward in only 10 weeks. Of course you may exercise for more than 30 minutes per day, but no more than 30 minutes per day will be counted toward your goal.

You must track your exercise online though a tracking tool available from your PHR, located at your medical carrier’s Web site. You are responsible for ensuring that data you enter in your tracking tool is accurate, based on the “honor system”.   

  • If your medical coverage is through Aetna or Kaiser, log on to www.Aetna.com and click on View Your Personal Health Record, then My Health Tracker to locate the Physical Activity Tracker.  
  • If your medical coverage is through UnitedHealthcare, log on to www.MyUHC.com and the Personal Health Center tab, and click on Track Your Health, then Exercise.

Obtaining your 2nd health reward after completing a defined exercise program
If your medical coverage is through Aetna or Kaiser, your second health reward will be processed by the Wellness Program Administrator when the exercise requirements for completion are accomplished, as long as you have completed the Health Assessment.

If your medical coverage is administered by UnitedHealthcare, you will receive a notification by mail or email that you have earned the second health reward. Once you respond, your health reward will be processed by the Wellness Program Administrator as long as you have completed the Health Assessment.

Please log your activity online as soon as possible, but no later than December 31, 2012. (Note: Any exercise logged after December 31, 2012, will not be counted toward the requirements for a second 2012 health reward.) Once you have logged at least 1,200 minutes, as described above, your second health reward will be processed. 

Visa Debit Cards
Please allow up to 60 days for the Wellness Program Administrator to process your health reward. Once processing is complete, Visa Debit Cards earned through the Program are distributed monthly.

If you haven’t received your card
If you have completed the requirements to earn a health reward but haven’t received your card, review the information above to ensure you’ve followed the instructions for obtaining your reward. And remember that you must complete your Health Assessment before you are eligible to receive any reward(s). 

  • If your coverage is through Aetna or Kaiser, go online at www.Aetna.com and check the Aetna Physical Activity Tracker to see whether the activity is indicated as completed. If it is, contact Aetna Member Services at 1-800-435-7324. If your Physical Activity Tracker does not indicate the activity was completed, check again when the site is updated on the first Wednesday of each month.
  • If your medical coverage is through UnitedHealthcare, contact the Health and Wellness customer service support line at 1-866-868-5484.

If you have lost your card 
If you have received your Visa Debit Card but then lost it, neither your medical carrier nor UPS can assist you with this process once the card has been issued. To obtain a replacement card, you must call:  

  • 1-866-490-0650 if you completed your Health Assessment at the Aetna Web site, or

  • 1-866-868-5484 if you completed your Health Assessment at the UnitedHealthcare Web site.

Taxable income
Health rewards earned by a covered employee and/or spouse will be added to the UPS employee’s income and taxed, as reported on the employee’s paycheck and annual W2.
